Form 1095-A (Health Insurance Marketplace Statement)
- Required if you or anyone on your tax return had health coverage through the Marketplace (Obamacare).
- Important: We cannot file your return without this form if you had Marketplace coverage. Please check your mail or log in to Healthcare.gov to print it.
Identity Protection PIN (IP PIN)
- Required if the IRS has assigned you a 6-digit IP PIN to prevent identity theft.
- Important: If you have an IP PIN, your tax return will be rejected if this number is missing or incorrect. Please bring your CP01A Notice (the letter from the IRS containing your PIN) or retrieve your PIN online at IRS.gov.
Tax Credits
- Child Care Provider/Address and Employer Identification Number (EIN) or Social Security Number (SSN)
- Adoption Expenses
- Retirement Savings Contributions Credit
